HOW DO YOU KNOW ABOUT US

I agree for Smart Dental Compliance & Training (Company Number: 11714494 - Smart Compliance Training LTD) to provide a software to manage my dental compliance (relevant to the package of choice) and ensure all the compliance processes and systems are developed in line with CQC regulations. This will also include all advisory support to your practice team.
Contract Agreement:
 
Contract Duration: The length of the contract is 12 months initially, after which you would automatically be tied into another 12 months contracts. After the 12 months you can downgrade or upgrade into different packages, however we require three months’ notice to downgrade or upgrade.

Signed Contracts: By inputting your direct debit details, you provide the consent and digital signature to enter within this agreement. Please note we do not require physical signatures.

By ticking the box which contains the terms and condition, you have provided consent and bared a signature.

Termination & Notice: Should you wish to terminate the agreement, we require written notice a minimum of 3 months before the 12-month period is up, if no notice is given, you will enter into another 12 months agreement. If you wish to terminate your contract within the 12 months, all remaining months must be paid. If you wish to cancel the contract before the 12 months, you are still liable to pay the remaining outstanding month payment.

Direct Debit: The first payment will leave your account 5 days from the date of sign up. If a direct debit is cancelled before the completion of your 12-month contract, you are required to pay the remaining balance up to 12 months.
 
Debt collectors – If DDs are cancelled without an agreement with the company, notice or payments are failed to be made your information will be passed onto the debt collection team. If there is a reason you cannot make payment, please ensure you inform us at your earliest convenience.


Package: Smart Manage Terms and Conditions
 
The smart manage package provides the following:
 
  1. Advisory support provided with a dedicated CQC Consultant - to include email/telephone responses to any staff member
  2. Development of all Practice Compliance documentation through the AIOM software
  3. All updates of any changes to regulations/legislation and new policies
  4. Access to All-In-one Management Software compliance tab, templates and uploads to manage and maintain your compliance with the support of Smart Dental Compliance and Training consultant.
  5. HR Employment Support & HR Dashboard – unlimited users within your practice
  6. CPD Platform
  7. CQC Mock Inspection Audit for each site annually, this is provided remotely
  8. Stock Management feature
  9. Lab Management feature
  10. Onboarding Site Visit, dependent on the location
  11. 10% off on all our equipment services
  12. 10% off on all our risk assessment services
  13. 10% on all our in house CPD Courses such as BLS
  14. Training Support


Package: Smart Consult Terms and Conditions
 
  1. Advisory support provided with a dedicated CQC Consultant - to include email/telephone responses to any staff member

  • 2 Consultancy Days – Experienced Qualified Consultant which will include the following
  • Completion of all Audits
  • Customisation of delegation of all policies and procedure
  • HR Staff Files Management (Adding documents and updating)
  • Organisation of all external service such as risk assessment, RPA, services
  • Organisation of Practice Meetings
  • Annual Mock Inspection
  • On day CQC Inspection Support

Package: Platinum Compliance Package Terms and Conditions
 
  1. Advisory support provided with a dedicated CQC Consultant - to include email/telephone responses to any staff member
  2. Development of all Practice Compliance documentation through the AIOM software
  3. All updates of any changes to regulations/legislation and new policies
  4. Access to All-In-one Management Software compliance tab, templates and uploads to manage and maintain your compliance with the support of Smart Dental Compliance and Training consultant.
  5. HR Employment Support & HR Dashboard – unlimited users within your practice
  6. CPD Platform
  7. CQC Mock Inspection Audit for each site annually, this is provided remotely
  8. Stock Management feature
  9. Lab Management feature
  10. Onboarding Site Visit, dependent on the location
  11. Training Support
Additional services within the package
  • Fire Risk Assessment
  • Legionella Risk Assessment
  • Health & Safety Risk Assessment
  • Disability Risk Assessment
  • PAT Testing ( This only includes 100 appliances, any additional appliance would be charged to the practice)
  • 1 Autoclave PVI Testing
  • 1 Compressor PVI Testing
  • RPA Service (includes, dedicated RPA, RPA folder and documentation through the online portal system)
Please note you can only book the above services after 3 months into the contract, all services must be booked throughout the year, rather than at once. All services which are included will only be carried out when due. Any site visits would be charged at additional cost, unless you are on the Smart Consult Package.


Data Protection:
The terms controller, processor, data subject, personal data, personal data breach and processing shall have the meaning given to them in the UK GDPR which has the meaning given to it in section 3 - 10 (as supplemented by section 205 - 4 of the Data Protection Act 2018.

We shall process the personal data we receive from you as processor on your behalf. You consent to (and shall procure all required consents, from your personnel representative and agents, in respect of all actions taken by us in connection with the processing of all personal data which we process in connection with this agreement.
You will ensure that you have all necessary appropriate consents and notices in place to enable lawful transfer of any personal data to us and the lawful collection of the same by us for the duration.
Processor and (Smart Dental Compliance) shall notify Company without undue delay upon Processor becoming aware of a Personal Data Breach affecting Company Personal Data, providing company with sufficient information to allow the Company to meet any obligations to report or inform Data Subjects of the Personal Data Breach under the Data Protection Laws.
